Supported Living Services (SLS) are designed to help individuals with disabilities live as independently as possible in their own homes and communities. These services provide personalized support tailored to the individual’s unique needs, including assistance with daily living tasks, life skills development, household management, health and wellness, and community integration. SLS promotes self-reliance and independence by empowering individuals to take control of their daily routines while offering guidance and support in areas such as budgeting, meal planning, personal care, and social engagement. The goal of SLS is to enhance quality of life, ensuring individuals can live fulfilling and meaningful lives while maintaining dignity and autonomy.

Personal Care Independence– Supporting individuals in developing the skills needed to manage their own personal care routines, encouraging self-reliance in daily tasks and hygiene.
Life Skills Development – Support with essential daily living skills, including managing household tasks, meal planning, and grocery shopping, to promote greater independence in home management.
Health and Wellness Support – Encouraging healthy habits and facilitating access to healthcare, medication management, and physical well-being to ensure individuals can maintain their health with minimal support.
Scheduling and Transportation – Support in organizing appointments and arranging transportation to maintain community engagement and independence.
Community Integration and Social Activities – Encouraging participation in social activities and local events to enhance community involvement.

We work closely with clients, their families, and support networks to create a personalized plan that addresses individual goals and needs.
This includes:
Goal Setting and Progress Tracking – Regularly reviewing and adjusting goals to reflect growth, changing needs, or evolving aspirations.
Involvement of Family & Other Support Networks – Active involvement of families and support networks in the planning process.
Entrance Criteria

Supported Living Services (SLS) is provided to an individual that meets the following criteria.
Male or female, ages 18-59 yrs with an ambulatory or non-ambulatory status. 60+ years of age must have Regional Center prior approval.
Must have diagnosis of a Mild to Moderate developmental disability per Regional Center's guidelines.
Individual must require supervision and assistance for behavioral excesses as well as for moderate deficits in self-help skills and adaptive functioning.
LHLS will consider accepting individuals who have medical conditions that do NOT require a 24 hour nursing care.
LHLS will consider individuals transitioning from a State Developmental Centers or a locked facility to a Supported Living Environment.
LHLS shall review the individuals circumstances with the Regional Center on a case-by-case basis

Step 1: Initial Contact
• What Happens: The first step is reaching out to the regional center or Lighthouse Living Services to express interest in receiving Supported Living Services (SLS).
• How to Start: Contact the regional center via phone, email, or through their online portal to initiate the referral process.
Step 2: Eligibility Determination
• What Happens: A regional center staff member will assess whether the individual meets the eligibility criteria for services under the Lanterman Developmental Disabilities Services Act (Lanterman Act).
• How to Start: You may need to provide documentation, such as medical records or evaluations, to confirm eligibility.
Step 3: Service Coordinator Assignment
• What Happens: If eligible, a Service Coordinator will be assigned to the individual. This person will guide you through the process and act as a primary point of contact.
• How to Start: The Service Coordinator will schedule an initial meeting to discuss the individual’s needs and goals for Supported Living Services.
Step 4: Needs Assessment
• What Happens: A comprehensive assessment is conducted to understand the individual’s unique needs, goals, and preferences. This includes evaluating their daily living skills, health requirements, and social engagement needs.
• How to Start: Work closely with your Service Coordinator to gather relevant information, including assessments, medical records, and personal preferences.
Step 5: Person-Centered Planning
• What Happens: Based on the assessment, a Person-Centered Plan (PCP) will be developed, outlining the specific services and supports the individual will receive. This plan reflects the individual’s goals, preferences, and desired outcomes.
• How to Start: Participate in meetings with the Service Coordinator to discuss and refine the plan.
Step 6: Referral to Supported Living Services
• What Happens: If the individual’s needs align with Supported Living Services, the Service Coordinator will formally refer the individual to Lighthouse Living Services or another approved provider.
• How to Start: Once referred, the individual will meet with a representative from Lighthouse Living Services to finalize the details and begin the service.
Step 7: Service Implementation
• What Happens: Once services are agreed upon, an action plan is created, and support begins. This can include assistance with daily living activities, skill-building, healthcare coordination, and community integration.
• How to Start: Start receiving services according to the agreed-upon plan, with regular check-ins to assess progress and make any necessary adjustments.
Step 8: Ongoing Support and Monitoring
• What Happens: Throughout the service period, the Service Coordinator and support team will continue to monitor progress and make any needed adjustments to ensure the individual is receiving the best care.
• How to Start: Participate in regular meetings to review the individual’s progress, update goals, and adjust services as needed.

How to Refer a New Client
Referring a new client to Lighthouse Living Services is simple. Whether you are a family member, caregiver, or healthcare professional, we’re here to guide you through the process. Follow these steps to begin:

• Reach out to our team to express interest in our services. You can contact us through our phone number, email, or online contact form. We’ll respond promptly to discuss your needs.
2. Complete the Referral Form:
• To get started, you will need to complete our New Client Referral Form. This form asks for basic information about the client, including their contact details, medical background, and the specific services needed. It helps us understand the individual’s unique needs to provide the best support.
3. Submit Documentation:
• Depending on the individual’s situation, we may request additional documents, such as medical records, assessments, or other relevant paperwork to confirm eligibility for services.
4. Initial Consultation:
• Once we receive the referral and necessary documentation, we’ll schedule an initial consultation to discuss the client’s needs, goals, and how our services can help. This consultation may be in person or via phone/video, depending on availability.
5. Develop a Person-Centered Plan:
• After the consultation, we’ll work together to develop a personalized plan based on the individual’s goals and preferences. This plan will outline the services provided, frequency of support, and any specific needs or preferences.
6. Start Services:
• Once the plan is agreed upon, we’ll begin providing the necessary services and support to help the individual achieve greater independence. Our team will be there every step of the way, offering guidance, assistance, and regular check-ins.

Admission:
Regional Center Service Coordinators: to submit a referral, please fax the referral packet to our offices. For more information about vendorization, visit our Contact page.
The admissions office will review the submission, and the admissions manager will contact the Regional Center to discuss the referral and subsequent intake meeting.
After receiving admission approval, the Regional Center will collaborate with the admissions coordinator at Lighthouse Living Services to secure funding approval before placement.
